Jokes aside, it was common for men in this era of history to specifically avoid seeking help for their mental health. As tragic as the results are, it was normal for men like Joe to drink away their problems rather than develop the skills and coping mechanisms that could lead to better outcomes for them. As Andre noted, it would have been seen as a sign of weakness if either Joe or Vito sought a licensed therapist.
While that stigma is being active dispelled in the modern day, there’s the added problem of health insurance in America. Even should one be willing to seek help, it is possible that they simply cannot afford to.
The problem is deeply complex, and Joe is demonstrating why it is a vital problem to solve even now. Had he had access, and had the stigma not been around, that bartender might still be alive.
